INSTRUCTIONS
Please choose the appropriate registration category. If you are a MOAPA member, you must log in to receive the member rate. If your employer is paying by check, complete and submit the registration form online. Make the check payable to “MOAPA" and mail to 222 S. Westmonte Drive, Ste. 111, Altamonte Springs, FL 32714. Please remit payment within a week after registration. A delay may result in the deletion of your registration.
*If you are retired, we are offering a discounted registration for all retirees.

CANCELLATION AND REFUND POLICY
A full refund is available if a written request for a cancellation is received by June 11, 2025. After June 11, 2025, a $75 fee will be assessed. *(Requests for cancellations must be approved by the conference committee.)
If you have not paid you will be receiving an invoice within 3 weeks of registering. Please pay via Credit Card. If you are mailing a check, please do so within 2 weeks of registering to avoid cancellation of your registration. Make the check payable to “MOAPA" and mail to 222 S. Westmonte Drive, Ste. 111, Altamonte Springs, FL 32714.
