INSTRUCTIONS

Please choose the appropriate registration category. If you are a MOAPA member, you must log in to receive the member rate. If your employer is paying by check, complete and submit the registration form online. Make the check payable to “MOAPA" and mail to 222 S. Westmonte Drive, Ste. 111, Altamonte Springs, FL 32714. Please remit payment within a week after registration. A delay may result in the deletion of your registration.

CANCELLATION AND REFUND POLICY

A full refund is available if a written request for a cancellation is received by the MOAPA office by September 19, 2022. After this a $75 fee will be assessed. (Requests for cancellations must be approved by the conference committee.)
